The Westchester Suzuki Institute

1. Complete the entire registration form or fill out online.

    Students aged up to 11 registration form:

    Tween program (students aged 12 - 15 years old and level) registration form:

    Tween program Waiver and Medical Information Forms

    (this form only necessary for 12-15 year old

     tween students)

2. Mail the registration form together with a check covering all fees or fill out form and pay online.
Checks should be made payable to HUDSON RIVER SCHOOL OF MUSIC.

3. Registration must be filed or postmarked on or before May 15, 2022.
   You must include a $50.00 Late Registration Fee after that date.

4. You will receive a confirmation of registration, along with further information by June 1, 2022.

 

Refund Policy   A full refund, less registration fee, will be made for all requests received in writing by April 1, 2022. No refunds will be made after that date under any circumstances.

Enrollment is limited. Acceptance will be on a first come, first served basis.
